THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Legal Duty
We are required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of Privacy Practices. PHI includes any information that identifies you and relates to your past, present, or future physical or mental health condition, treatment, or payment for treatment.
We are required to abide by the terms of this Notice currently in effect and to notify you following a breach of unsecured PHI.

Treatment

- Psychotherapy notes (a clinician’s personal notes about a therapy session) are afforded the highest level of protection and require your specific authorization for disclosure, except in limited circumstances
- Information about substance use disorder treatment, HIV/AIDS status, and domestic violence is subject to heightened protection under applicable federal and state laws
Uses and Disclosures Requiring Your Written Authorization
Other than the exceptions listed above, we will only use or disclose your PHI with your written authorization. You have the right to revoke your authorization at any time, in writing. Revocation will not affect uses and disclosures already made in reliance on your authorization.
Your Rights Regarding Your Health Information
You have the following rights with respect to your PHI:
- Right to inspect and copy: You have the right to inspect and receive a copy of your health information, with limited exceptions. We may charge a reasonable fee for copies.
- Right to amend: You have the right to request amendment of your health information if you believe it is incorrect or incomplete. We may deny your request in certain circumstances.
- Right to an accounting of disclosures: You have the right to receive an accounting of certain disclosures we have made of your PHI during the past six years.
- Right to request restrictions: You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requested restrictions.
- Right to request confidential communications: You have the right to request that we communicate with you through alternative means or at alternative locations.
- Right to a paper copy of this notice: You have the right to receive a paper copy of this Notice of Privacy Practices upon request.
- Right to be notified of a breach: You have the right to be notified if a breach of your unsecured PHI occurs.

Part 3: Good Faith Estimate: Your Rights Under the No Surprises Act
Under the law (Section 2799B-6 of the Public Health Service Act, effective January 1, 2022), healthcare providers, including mental health providers, are required to give uninsured or self-pay patients a Good Faith Estimate of the cost of services before those services are provided.
What Is a Good Faith Estimate?
A Good Faith Estimate is a written statement of the expected costs of non-emergency health care services, including mental health services. It is provided so you can make an informed decision about your care.
Who Receives a Good Faith Estimate?
You have the right to receive a Good Faith Estimate if:
- You are uninsured (do not have health insurance), or
- You choose to pay out-of-pocket (self-pay) and do not plan to use your insurance for our services
What Will Be Included in Your Good Faith Estimate?
Before you begin services, we will provide you with a written Good Faith Estimate that includes:
- The expected cost of each service or item
- Service codes (CPT/ICD-10 codes) for the services
- Provider name and National Provider Identifier (NPI)
- Facility name and NPI (if applicable)
- Diagnosis codes expected for the course of treatment
- Expected number of sessions or treatment duration (to the best of our ability to estimate)
Our Standard Session Fees
Our current standard fees for self-pay clients are provided during your free 15-minute initial consultation. Fees are subject to change; any changes will be communicated to you with reasonable advance notice. A personalized Good Faith Estimate will be provided before your first paid session.
Your Protections Under the No Surprises Act
- You have the right to receive a Good Faith Estimate in writing at least 1 business day before your scheduled service (if the service is scheduled at least 3 business days in advance)
- Make sure to save a copy or take a photo of your Good Faith Estimate
- If your final bill is at least $400 more than your Good Faith Estimate for any provider or facility, you can dispute the bill

The Good Faith Estimate is not a contract and does not require you to obtain the listed services or obligate you to pay the estimated amounts. Actual costs may differ based on the services you receive.

Part 4: Telehealth Informed Consent
All of our services are provided via telehealth (video-based therapy). Before beginning telehealth services, clients are required to complete a formal Informed Consent for Telehealth document during the intake process. The following provides a summary of key information about our telehealth services.
What Is Telehealth?
Telehealth refers to the delivery of mental health services using electronic communication technologies, including secure video conferencing. We provide telehealth services to individuals and couples located in New York State.
Benefits of Telehealth
- Convenient access to care from the comfort and privacy of your home or other location
- Eliminates travel time and geographic barriers
- Increased scheduling flexibility
- Continuity of care during illness, inclement weather, or other circumstances that prevent in-person attendance
Risks and Limitations of Telehealth
While telehealth is effective and clinically appropriate for most couples therapy and individual therapy services, there are some risks and limitations to be aware of:
- Technology failures: Internet interruptions, power outages, or technical difficulties can disrupt a session
- Confidentiality risks: Sessions must be conducted in a private location; therapists cannot control the security of your internet connection or home environment
- Clinical limitations: Telehealth may not be appropriate for all clinical presentations, particularly those involving active crisis, severe psychiatric conditions, or needs that require in-person assessment or intervention
- Emergencies: In the event of a mental health emergency during a telehealth session, your therapist may need to contact local emergency services at your location. It is essential that you provide a current address and emergency contact before beginning services.
Confidentiality in Telehealth
We use HIPAA-compliant video platforms for all telehealth sessions. Our clinicians conduct sessions from private, secure locations. You are responsible for ensuring your location is private during sessions. Do not conduct telehealth sessions from public spaces, shared work environments, or any location where you may be overheard without your consent.
Licensure and Jurisdiction
Our clinicians are licensed in New York State. Telehealth services are provided to clients located in New York State at the time of service. If you travel outside of New York, please notify your therapist before your session, as licensure laws vary by state and may affect your therapist’s ability to provide services while you are in another jurisdiction.
Technology Requirements
To participate in telehealth sessions, you will need:
- A device with a camera, microphone, and speaker (computer, tablet, or smartphone)
- A reliable internet connection (minimum 5 Mbps recommended for video quality)
- A private, quiet location free from interruptions
- The video platform link provided by your therapist
Emergency Protocol
Telehealth is not appropriate for mental health emergencies. If you are experiencing a psychiatric emergency, including thoughts of suicide, self-harm, or harm to others, please:

Your therapist will discuss emergency protocols with you during your intake and will obtain your current address and emergency contact before beginning services.
